This a monthly commitment being made Month to Month. The customer is responsible for completing the authorized contract. Payment of 120.00 (before taxes) will be due monthly. The contract will automatically be renewed after the month is completed. Please inform staff if you do not want the contract renewed.

The Customers form of payment will be stored and authorized to use monthly by the system, if the payment is rejected a processing fee will be charged.
