12 Month Contract
This a monthly commitment being made for 12 months. The customer is responsible for completing the authorized contract. Termination of the contract can be negotiated due to injury or life-changing events. Documents will need to be provided to terminate the contract. Payment of 80.00 (before taxes) will be due monthly for 12 months. The contract will automatically be renewed after the 12 months is completed. Please inform staff if you do not want the contract renewed.

The Customers form of payment will be stored and authorized to use monthly by the system, if the payment is rejected a processing fee will be charged.
