Conquer Kickboxing (6 Month Contract)
This a monthly commitment being made for 6 months. The customer is responsible for completing the authorized contract. Termination of the contract can be negotiated due to injury or life-changing events. Documents will need to be provided to terminate the contract. Payment of 100.00 (before taxes) will be due monthly for 6 months. The contract will automatically be renewed after the 6 months is completed. Please inform staff if you do not want the contract renewed.

The Customers form of payment will be stored and authorized to use monthly by the system, if the payment is rejected a processing fee will be charged.

    Terms and conditions
    This membership bills every month. The first payment is due on the membership's start date. This membership entitles the purchaser to a full or partial discount on events/classes offered by the business. Some classes/events may be excluded from this membership. Unless otherwise noted, this membership is non-transferable. Your payment details will be securely stored in our system for the duration of the membership. This membership cannot be cancelled early. The full 6 months must be purchased to receive the rate listed.
